Hives with facial swelling, throat tightness, or any breathing change is anaphylaxis territory — 911. A rash that looks like bruising or tiny blood spots and doesn't blanch when pressed, especially with fever, is an emergency evaluation. Rapidly spreading, severely painful skin with fever also goes straight in.
