Immigration Medical Exams (USCIS Form I-693)

Most people applying for a green card from inside the United States (adjustment of status) need an immigration medical exam, documented on Form I-693. USCIS accepts the form only from a designated civil surgeon, so confirm the doctor's designation before you book.

Only a civil surgeon can do it

With narrow exceptions, only physicians designated by USCIS as civil surgeons may perform the exam and complete Form I-693 in the U.S. Use USCIS's Find a Civil Surgeon tool, or call the USCIS Contact Center at 800-375-5283. A clinic that lists immigration exams should be able to tell you which of its physicians holds the designation.

Preparing for the visit

Print the current edition of Form I-693 and bring it with your medical and vaccination records. Complete Part 1, but don't sign until the civil surgeon tells you to. The civil surgeon completes and signs the form and gives it to you in a sealed envelope. Don't accept it unsealed. Since December 2, 2024, USCIS requires Form I-693 to be submitted with Form I-485. If you file by mail, send the envelope sealed. If you file online, follow USCIS's instructions to upload the form, and keep the original and envelope.

What it costs

USCIS doesn't regulate civil surgeon fees. Rates vary, many civil surgeons don't accept insurance, and insurance may not cover much of the exam, so call several and compare. See the full Illinois cost guide.
